Scheduling Requirements:

To maintain membership benefits, fills must be scheduled every 2–3 weeks. If more than 4 weeks pass between appointments or retention is below 40%, an upgrade fee or new full set may be required.

Membership Terms:

3-month minimum commitment required
No rollovers or refunds for unused fills
After the initial 3 months, the membership continues month-to-month
14-day notice required to cancel before the next billing cycle
